Wastomi Suhari, Head of the Surabaya Land Transportation Organization (Organda), revealed a written request for a visum et repertum addressed to the Community Health Center (Puskesmas) in Sedati, Sidoarjo. Dated July 13, 2013, and signed by the subdistrict police chief, the visum is needed as evidence in a complaint lodged by Semedi, a 43-year-old Orenz Taxi field supervisor in Surabaya.
